A number of organizations prefer to pay a fee since it is easier for them to pay an invoice than to go through their Boards to generate a donation. Others prefer to make their payment in the form of a tax-deductible donation. The form generated below can be used for either payment method.

Fee waiver accounts: only accounts that have previously been approved for a fee waiver may choose the fee waiver renewal option. Fee waiver accounts have a uniform renewal date of January 1. We reserve the right to request resubmission of fee waiver applications.
