| Section A (Dues Deduction
Authorization) of the Membership Form: |
| This method of dues payment
authorizes the automatic monthly deduction of $2.50 by SURS from their
payments to annuitants or survivors. It alleviates the need to remember
to renew membership annually. The membership year begins July 1. |

| Section B ( Annual
Dues Payment) of the Membership Form: |
|
This method of dues payment requires a
check in the amount of $30.00 payable to SUAA/UIUC. It is necessary
for all members who: a) do not receive monthly checks from SURS or b)
who wish to make annual renewals and payments.
Of these dues, $20.00 supports the SUAA Central (State)
Office in Springfield and its many activities on behalf of all SURS
annuitants and survivors. The remainder is retained by the UIUC Chapter
to support local activities.

| Inside Illinois Subscription
Form: |
| A subscription to receive
Inside Illinois by mail for $7.00 per year
may be requested by completing the form. The subscription is available
to current members, begins in August with the new UIUC year, and continues
through the following July. Those who elected the Section A (Dues Deduction)
method of membership payment will need to enclose a check in the amount
of $7.00 payable to SUAA/UIUC Chapter. Those who elected the Section
B (Annual Dues Payment) method of membership payment may write one check
in the amount of $37.00 for the combination of membership and subscription. |
