- In person contact with child in substitute care both intial and subsequent placements.

- In person visit to children who remain at home and complete CERAP

- Weekly supervision
- Twice monthly in-person contact with child ( more often if child has special needs)
- Monthly in-person contact with substitute caregiver
- Weekly in-person contact with parent (and any children that remains with the parent)
- Monthly contact with service provider

First 45 days

- Participate in all activities to ensure Integrated Assessment is complete
- Conduct first family meeting

- Weekly in-person contact with child

30 days through achievement of permanency and case closure

- Monthly supervision
- Monthly in-person contact with child unless child has special needs that require more frequent contact
- Monthly in-person contact with substitute caregiver
- Monthly in-person contact with parent (unless parental rights are terminated)
- Monthly contact with service providers
- Conduct at least quarterly family meetings with Child and Family Team
- Twice yearly contact with child's teacher
- Ongoing contact and coordination with public health nurse for SEI infants

- Same requirements as first 30 days and 30 days until achievement of permanency
- During a pending investigation, weekly monitoring visits with involved children unless these visits are being done by the investigator.
