3.11 APPENDIX D: Assessment Summary

The assessment consists of the following instruments completed by the specified worker:

• Completed by the child protection services worker during the investigation:
• CERAP: within 24 hours of seeing the child victim(s)
• Risk Assessment Protocol, using the Expanded Risk Factor Examples and the Indicators as guides: prior to completing the investigation or, if case is to be opened, prior to the handoff.
• Completed by the permanency worker after the handoff:
• CERAP: within five days of seeing the family
• Risk Assessment Protocol, using the Expanded Risk Factor Examples and the Indicators as guides: within 30 days of case opening
• Social History: within 30 days of case opening.
• Additional assessment activities
• When children are in placement

* Initial health screening and registration with Healthworks (if not already done by the CPSW)

* Compilation of medical and immunization history

* Comprehensive medical exam within 21 days of protective custody

* Diligent search for missing or absent parents

* Basic education screening

* Assessment of the appropriateness of the child's current placement

Completion of the comprehensive assessment within 45 days of case opening.
