We can send you an application or you can come in and pick one up. You can complete the application in our lobby if you would like. Our staff is more than willing to help you complete the application.

Once you have submitted an application, we will send you an information packet which contains your appointment time, a list of the documentation you will be required to bring to your intake appointment, and a few forms that must be completed.

Once you have submitted all necessary information, the Township staff can make the necessary verifications and determine your eligibility.

By policy we have 30 days from the date of your application to make a decision; however, we are usually able to make determinations within a shorter time period.
